Needed:

·        Player and one parent must be present to sign all registration forms

·        $550.00 non refundable check written to Jumpstart Sports Club    

·        Three completed forms from NCVA website and two from the Jumpstart Registration Packet given at tryouts:

1.         NCVA Individual Membership Form

2.         Medical History and Release Form

3.         NCVA Letter of Commitment

4.    Jumpstart Contact Information Form (in the registration packet given at tryouts) or Click here for copy

5.    Jumpstart Letter of Intent (In the registration packet given at tryouts)

Directions

for forms:

  Go to NCVA website www.ncva.com

1.      Enter the “2009-2010 Registration” tab to register your daughter with NCVA. 

2.      Select "Jr Girls Player" under the heading Girls League.

3.      Select “Register Now" under the paragraph that informs you that your daughter will register for No Club Affiliation.

4.      The next screen is "Register Now."  You have two options, Existing Members or New Registration.

5a. If you are an Existing Member follow the prompts until you reach the "Complete Registration:" section.  You will need to select either, "Register & Pay Later" or Register & Pay Online."  You need to select "Pay Online" because you will need to pay and later send an email NCVA indicating you want to register with Jumpstart. Go to step 12 for a list of forms that need to be signed.

5b.The following instructions are for new members. If you select "New Member" under the New Registration.  Fill out information regarding Parent/Guardian Contract Infomration. <Next>

6.      Enter Email and Password information, <Next>.

7.   The next section is "New Child Profile," fill out your child's information then press <Next>

8.   The next section is the Jr. Girls Membership Fee, select "Add." then <Next>

9.   The next section is "Registratioin & Member System," fill out the information <Next>

10. Put your initials at the bottom of the Member Summary Information and select <Next>

11.To complete the registration, select “Register & Pay on Line.” You need to pay with a credit card or online check.  At a later time, you will need to send NCVA a letter indicating to move your registration to Jumpstart. 

12.At the "Thank you for completing the NCVA application" section there will be a list of forms that can be printed.  You need to print the following three NCVA forms:

      1.  2009-10 Girls Letter of Commitment

      2.  2009-10 Medical Form (Jr. Girls)

      3.  2009-10 Membership Form (Jr. Girls)

9.      Bring these forms to the signups on Monday, November 9 at 6:00 p.m. to Buchser Middle school.
